And research that could change how a chronic or rare condition is managed often takes years to reach routine care. Nobody is at fault. Nobody has the whole thing.
Clarity360 is a health synthesis and engagement platform. It brings a complicated health history together with life context and readiness to engage so the individual can better understand what is happening, identify realistic priorities, and participate more effectively in what comes next.
The guided experience turns that synthesis into a Health Story, Action Plan, and Companion Guide for the individual and the people supporting them.
The information is often already there. Nothing puts it together.
One diagnosis may sit in one chart. Symptoms in another. Labs somewhere else. Add years of lived experience, several specialists, treatments, and what someone has learned about their own body, and it becomes difficult for anyone to see the whole story at once.
Most people navigating this have a version of the same story. A binder or a phone folder carried to every appointment. A history repeated so many times they've started editing it for time. A symptom they stopped mentioning because nobody had anywhere to put it.
Clinicians describe the other half. A new patient with nine years of complex health history, fifteen minutes, and a referral note that covers one organ system or a single diagnosis despite much more going on.
And plenty of people are doing this without a care team at all. Rural, uninsured, between systems, or simply without anyone taking the whole thing on.

Clarity360 uses a guided model so the synthesis stays grounded in the individual’s health information, lived experience, questions, readiness, and capacity. The process also gives us the opportunity to learn what is most useful to individuals and the organizations supporting them as the model grows.
How it works
STEP 1
Health, Context + Readiness
The intake process gathers health history, diagnoses, symptoms, medications and treatments, labs, family history, major health events, and lived experience.
It also captures life circumstances, social and environmental factors, practical constraints, health literacy, self-efficacy, readiness to act, and communication preferences. That context matters because a useful next step has to fit inside someone’s actual life.
STEP 2
Structured Synthesis
The individual’s health information is synthesized across physiological drivers, lifestyle levers, and a set of structured knowledge libraries, with evidence strength kept visible throughout. The aim is to surface connections that may be hard to see when information is split across conditions, symptoms, treatments, records, and lived experience.
Underlying labels stay behind the scenes. The individual receives plain language.
STEP 3
Insight Into Action
The synthesis becomes three personalized deliverables: My Health Story, My Action Plan, and a Companion Guide.
Together they help the individual understand the story, identify realistic priorities, and carry relevant context into the clinical or community support around them.

The Practitioner Edition of the Companion Guide translates relevant context and action priorities into plain language for coaches, navigators, community health workers, peer supporters, and other non-clinical supports without moving them into clinical interpretation.

Clarity360 can be useful even when someone does not have a diagnosis for every concern or an established care team. It is designed to work within the support available to the individual, including clinical, community-based, and non-clinical support, which may be especially important in rural and resource-limited settings.
The evidence isn't equally strong everywhere.
What’s established, what’s emerging, and what still needs exploring.
Clarity360 keeps evidence strength visible throughout the synthesis. Some connections are well established. Others are emerging or still thin but promising, particularly in women’s health, rare disease, and research that cuts across conditions.
Those limitations matter. They don’t mean potentially useful connections should automatically be ignored. The uncertainty stays visible while reasonable areas for exploration can still be considered.
pilot phase
Clarity360 is being used and refined through initial pilots.
Initial pilots may be conducted with clinician practices, community health organizations, advocacy organizations, or other partners serving people with complex health needs. The first cohorts will focus on women ages 40 to 60 and use three core touchpoints: intake clarification, delivery of the Health Story and Action Plan, and a 30-day follow-up.
Starting with a focused cohort gives us a practical way to learn where Clarity360 creates the most value, what people use, and how it fits alongside existing clinical and community support.
Why start here? Midlife women often face several layers of change at once across hormones, metabolism, sleep, stress, cardiovascular health, and autoimmune burden.
When symptoms cross systems and the evidence is fragmented, the challenge is not just understanding what may be connected. It is knowing what to focus on first.
